Phoenix-Talent Schools Athletics Registration
To participate in athletics, students must have a current sports physical and be registered online through FinalForms (2025-26 fall sports registration begins on July 1, 2025). Students can get sports physicals from their primary care provider, at the La Clinica School-Based Health Center at Phoenix High School or at the free sports physicals clinic June 21 at South Medford High School.
