SBHC open during summer

Great news for Phoenix High School families — La Clinica's School-Based Health Center at PHS remains open this summer, giving students continued access to the quality health care they count on during the school year.

La Clinica's school-based health centers bring comprehensive medical and mental health services directly to students — no need to find a separate provider or worry about coverage. In addition to serving Phoenix-Talent School District students, care is available to eligible youth and siblings throughout the community, even if they don't attend a PTS school. Students and community youth are eligible for services regardless of income or insurance status, and for many families, the clinic serves as an essential health care safety net.

Services available include diagnosis and treatment of illness, preventive care, sports physicals, immunizations, vision and hearing screenings, mental health counseling, and much more.

Clinic Office — Walk in anytime with questions or to schedule an appointment
Monday–Friday: 7:30 a.m.–4 p.m.

Medical Services
Monday & Wednesday: 10 a.m.–4 p.m.
Tuesday & Thursday: 9 a.m.–3 p.m.

What Families Should Know

Students under 15 need a signed consent form from a parent or guardian before being seen. Forms are available at the health center, the school's main office, or can be downloaded from La Clinica's website. Parents are always notified when their child visits the health center. Medical records are confidential and are never part of a student's school record. All care is coordinated with families and provided in accordance with state and federal law.

La Clinica will bill insurance for insured students. Most services are free for uninsured students whose families earn at or below 200% of the Federal Poverty Level. Staff can also help families apply for the Oregon Health Plan.
