School-Based Health Centers
Phoenix-Talent SD partners with La Clinica to bring the doctor's office into our schools. Students can be seen by a medical provider right when they need it and connect with ongoing behavioral health support in a familiar, convenient setting. Youth at all six of our schools are eligible for services regardless of income or insurance coverage. In addition to serving students, care is available to eligible youth and siblings throughout the community, even if they do not attend the school where the health center is located.

What families should know
Consent for your child's care
A parent or guardian must sign a consent form for children under 15 to get medical care and for those under 14 to get behavioral health services. Older students can consent to their own care. Forms are available at each school's health center and main office, or can be downloaded from La Clinica's website.
Privacy & health records
Health records are confidential and are not part of a student's school record. Parents who consent to care are notified when their child visits the health center. Care is coordinated with families and provided in accordance with state and federal law.
Insurance & billing
La Clinica bills insurance for insured students. Uninsured patients may qualify for discounted services through a sliding fee program, and many services may be available at low or no cost depending on income. Staff can help families apply for the Oregon Health Plan (OHP) and other health-coverage programs.
